Everyone but Meredith Is a Physical Embodiment of Emotions
One fan thinks Meredith has Alzheimer's disease and other characters are emotions, like in Inside Out. When a character leaves, the emotion goes with it. Derek is love, Arizona is happiness, Sloan is lust, Izzie is compassion, and so on.
Penny Is From Parks and Recreation's Pawnee
It's not uncommon that fans of different shows will find some overlap with actors, then theorize that the universes might intertwine. One fan pointed out that actress Samantha Sloyan was also on Parks and Recreation for one episode. Her character's name on that show is Erica, but we know Grey's Anatomy's Penny moves every two years with her mother. Why couldn't she have lived in Pawnee and maybe changed her name along the way?

Lexie Grey Never Existed
This is definitely a wild theory, mostly made due to the show's bad writing. The timelines for the supposed relationship between Susan and Thatcher that resulted in Lexie gets screwed up multiple times. She is supposedly born in 1984 and is a surgical intern in 2006. This would make her 22, which is way too young. This has led to a theory that she is just a figment of our imagination.

The Entire Show Is Zola Reading Meredith's Diary
One fan has come up with a slightly plausible theory of the show's ending — sadly, it doesn't have a good ending for Meredith. "What if Grey’s Anatomy ends with a diary being closed by a black girl’s hand," a fan wrote on Tumblr. "And the camera goes up to reveal a grown up Zola (who’s wearing intern scrubs), and it turns out Meredith developed and died of Alzheimer’s and wrote diaries like her mother did, and now Zola is carrying on the Grey legacy by becoming an aspiring surgeon at Grey Sloan Memorial Hospital?"
