I have always been very skiddish about asking nice restaurants for condiments. I work at a hotel with a 4-diamond restaurant, so I asked one of the waiters, and he said that it's ok to ask for ketchup, but not so much for A-1. It's all about the concept that if you are paying to eat at an upscale restaurant, you should trust the chef to prepare you the food to taste already. You also should not add salt to your food before you have had a taste, its insulting.

What about at a pizza parlor? They tend to give you an odd look when you douse your ham and pineapple pizza with hot sauce. ;)
