Tricia Miller

Tricia (Madeline Brewer) had been struggling with drug addiction since we were first introduced to her in Litchfield. But CO George "Pornstache" Mendez, notorious for being one of the worst guards of them all, makes sure to keep her under his control. He brings drugs into the prison, and had her sell it to other inmates. At first, it seems like she's all for it, but then she decides to really try to get clean and tells him she could no longer do his dirty work. As you would imagine, he doesn't like that answer very much, and forces her to do it anyway. Tricia ends up using some of the drugs she is supposed to sell. When CO Mendez notices, he locks her in a closet. When he came back, he finds her dead from an overdose. In an effort to cover his own tracks, he makes it look like a hanging.

At the end of season two, Miss Rosa (Barbara Rosenblat) is undergoing chemotherapy at a hospital outside of the prison walls every week. She had been battling cancer for quite some time, and we got a look into her treatment. When Miss Rosa goes to the hospital to talk with her doctor (with CO Charles Ford in the room), she finds out that chemotherapy is not working and she has a matter of weeks left. Miss Rosa gets back in the prison van with the CO Ford and Morello, the driver, to head back to Litchfield. But when they arrive, they find out that the prison is on lockdown because inmate Yvonne "Vee" Parker has escaped. The CO gets out of the van and tells the other two to stay there. So what does Lorna do? She convinces Miss Rosa to take the van and drive fast. And she does just that. So while we don't see her die, we never see Miss Rosa again and can only assume that her doctor was right.

Yvonne “Vee” Parker

Many people would probably agree that Vee (Lorraine Toussaint) gets exactly what's coming to her. She is a vicious woman with no moral conscience and a tendency for violence — even turning on her friends and family. Vee escapes the prison through a tunnel from the prison to the surrounding woods. She had beaten Red with a lock in a sock and is going to face the consequences. Vee makes her way to the road where she tries to flag down a van — little does she know that Miss Rosa is behind the wheel. Miss Rosa purposely swerves to the edge of the road to hit Vee with the van, making a comment about how rude Vee has always been.

Poussey Washington

Poussey (Samira Wiley) is sweet, funny, and leaves the series entirely too soon. In the second-to-last episode of season four, a peaceful protest in the cafeteria turns into chaos, and all COs are on deck to manage the situation. When Crazy Eyes starts having a psychotic break, Poussey tries to help, but CO Baxter Bayley throws her to the ground. At that point, Crazy Eyes started attacking him. As CO Bayley is fighting her off, he is also holding the back of Poussey's neck and has his knee on her back. She gasps for air and says that she can't breathe. She dies very quickly right there on the cafeteria floor. Her death causes the the riot that takes place over the entire fifth season.

CO Thomas Humphrey

All hell breaks loose in the finale of season four of OITNB. Humphrey (Michael Torpey) brings a gun into Litchfield against policy, and when he reaches for it during the riot, he drops it. Daya picks it up, and holds the gun on him as he's kneeling in front of her with his hands up in the air.

We're brought back to this moment in the premiere of season five. Dozens of inmates surround Daya and Humphrey, screaming at her about what she should do — many of them are saying to let him have it. As Humphrey begs for his life, he tries to relate with Daya by speaking in Spanish. Avid OITNB watchers know that Daya is basically the only Latina in the prison who doesn't know the language. She shoots Humphrey in the upper thigh and he falls immediately.

It seems like he is going to be okay for a while, but something happens when he's placed in a hospital bed between Crazy Eyes and Maureen. When Humphrey is asleep, Maureen asks if you can kill a person when they're already dead, and then proceeds to blow air bubbles in his IV. Humphrey has a stroke and then is paralyzed. A while later, Crazy Eyes discovers that he's no longer breathing.
